Vibram FiveFingers put each toe in its own pocket, which gives the most ground feel available and makes sizing genuinely difficult — toe length matters as much as foot length. These are also the shoes used in the 2013 study that found bone-marrow edema in 10 of 19 transitioning runners.

FiveFingers are the most recognizable product in this category and the least representative of it. Everything else on this site is a shoe with a wide front; these are gloves for your feet, and that changes the fit problem, the ground feel and the risk profile all at once.
Sizing: toe length matters as much as foot length
In any other shoe, your toes share a space and small differences in their relative lengths do not matter. In a FiveFinger, each toe has to reach the end of its own pocket — not past it, not well short of it. That means the thing being fitted is the position of each of your toes, not just the overall length of your foot.
Vibram publishes a dedicated sizing procedure requiring both your foot length and the position of your longest toe, and it exists because converting from a normal shoe size genuinely does not work here. Follow it exactly. Even then, expect to exchange once — that is not a failure, it is the normal experience with this product, and buying from somewhere with a workable returns process matters more than usual.
One further wrinkle: if your second toe is longer than your big toe, or if you have toes that curl, the pockets may never sit comfortably. That is a foot-shape constraint rather than a sizing one, and no size fixes it. Foot shape types.
The transition risk, which we are not going to downplay
The most-cited caution in this entire category comes from a 2013 MRI study of 36 runners, 19 of whom spent 10 weeks transitioning into minimal footwear. Ten of those 19 developed new bone-marrow edema, and most of them had no symptoms at the time. The shoes used in that study were FiveFingers.
That does not make them dangerous footwear; it makes a fast transition into near-zero-stack shoes a documented risk, and it means the absence of pain is not evidence that you are adapting safely. What they are genuinely excellent for
- Ground feel. Nothing else comes close, because nothing else lets your toes grip independently.
- Lifting and gym work. A flat, thin, stable platform with independent toe splay is close to ideal for a deadlift, and the stability comes from your foot rather than from the shoe.
- Water and rock. The KSO EVO and similar models drain, grip wet rock and stay on your foot in current, which is a genuinely hard combination to find.
- Proprioceptive training. If you are deliberately working on foot and ankle control, more sensory information is the point.

Toe-box width is only half the question — the other half is shape, because a square-toed foot in a tapered toe box is cramped at the little toe even when the shoe is genuinely wide. - Egyptian (tapered) — A toe box that tapers - but tapers to the OUTSIDE, following your diagonal, rather than squeezing toward a center point. In a square or symmetrically rounded toe box you get space you cannot use at the little toe while the big toe still reaches the end.
- Roman (even first three) — Width across the front third of the toe box specifically, with a generous big-toe allowance. A tapered toe box pushes toes two and three inward against the big toe, which is what starts the overlapping-toe problem.
- Square (near-equal toes) — A genuinely square, foot-shaped toe box with full width carried right to the front. Almost every conventional shoe fails this shape, and so do several 'wide' barefoot shoes whose width is in the midfoot rather than at the toes.
